Job summary

Walking In Life seeks a motivated Case Coordinator with a nutritionist or dietitian background to support an Australian allied health team from Malaysia. This office-based role focuses on client engagement and coordinating services under Australian standards, while collaborating with clinicians and support staff.

You will manage client onboarding, organize services with OT, Physiotherapy, and Speech Therapy, and help implement care plans with a strong emphasis on safety and wellbeing.

Qualifications

• Non-clinical case coordination experience preferred.
• Interest in nutrition or dietetics helpful.
• Ability to engage clients and families effectively.

Responsibilities

• Be the main contact person for clients, families, and carers.
• Help new clients get started by collecting basic personal and support information.
• Arrange and coordinate services with allied health professionals (OT, Physiotherapy, Speech Therapy, etc.).
• Assist with putting care and wellbeing plans into place and reviewing them over time.
• Check on client wellbeing, identify any concerns, and report issues when needed.
• Communicate with clinicians, counsellors, support coordinators, and community services.
• Organize appointments, schedules, and service arrangements.
• Keep clear and accurate records, notes, and reports.
