Job summary

WIL Health Coordination Malaysia Sdn Bhd is seeking a motivated Case Coordinator with a nutritionist or dietitian background to join our allied health team. You will work closely with an Australian allied health team, supporting high-quality, client-centered services under the Australian aged care framework, while being based locally.

Qualifications

• Background as a nutritionist or dietitian.
• Experience in case coordination and client engagement.
• Willingness to engage with an Australian aged care framework.

Responsibilities

• Be the main contact person for clients, families, and carers.
• Help new clients get started by collecting basic personal and support information.
• Arrange and coordinate services with allied health professionals (OT, Physiotherapy, Speech Therapy, etc.).
• Assist with putting care and wellbeing plans into place and reviewing them over time.
• Check on client wellbeing, identify any concerns, and report issues when needed.
• Communicate with clinicians, counsellors, support coordinators, and community services.
• Organize appointments, schedules, and service arrangements.
• Keep clear and accurate records, notes, and reports.
• Support safety, duty of care, and service quality standards.
