Volunteers of America Colorado is seeking a Part-time Case Manager to assist homeless veteran populations in Durango, CO. This role focuses on eligibility assessments, intakes, and coordinating services to secure housing and support resources.

The candidate will deliver education on employment, mental health, and housing resources, maintaining comprehensive case notes and participating in regular program meetings. A valid driver’s license and a willingness to travel are required.

Formación

• Bachelor’s degree or the equivalent in a related area.
• One year direct experience with vulnerable populations preferred.
• Own automobile, Colorado-mandated automobile insurance and a good driving record required.

Responsabilidades

• Assess eligibility, conduct intakes, and coordinate services for veterans in the Grant Per Diem program.
• Provide case management and crisis intervention as needed; maintain case notes and reports.
• Collaborate with VA and other partners to determine benefits and housing options.
