Job summary

Placements24 is seeking a Clinical Pharmacist (Pharm.D.) in Querétaro City, MX to provide advanced pharmaceutical care in a hospital setting. Qualifications

• Doctorate in Pharmacy (Pharm.D.) or equivalent from an accredited institution.
• Licensed Pharmacist in the state of Querétaro, Mexico.

• At least 3 years of progressive experience in clinical pharmacy, preferably in an acute care setting.
• Strong understanding of pharmacotherapy, clinical pharmacology, and evidence-based medicine.
• Excellent communication and presentation skills, with the ability to interact effectively with diverse healthcare professionals and patients.
• Proficiency in using electronic health records (EHR) and pharmacy information systems.
• Demonstrated ability to work collaboratively within a healthcare team.
• Board certification in a relevant specialty (e.g., BCPS, BCPP) is a strong asset.

Responsibilities

• Conduct medication reconciliation for patients upon admission, transfer, and discharge.
• Participate in interdisciplinary rounds to provide drug therapy recommendations.
• Monitor patients for therapeutic effectiveness, adverse drug reactions, and drug interactions.
• Educate patients and healthcare providers on appropriate medication use.
• Develop and implement clinical pharmacy protocols and guidelines.
