Job summary

Advocate Health seeks a licensed Social Worker to support hospice patients and families across Manitowoc and Green Bay territories. You will perform psychosocial assessments, counseling, bereavement support, and collaborate with an interdisciplinary team to develop and implement plans of care.

This full-time role requires a Master’s in Social Work and Wisconsin licensure, with duties including on-call responsibilities and community referrals in a compassionate hospice setting.

Qualifications

Master's degree in Social Work from an accredited school.
One year of medical clinical or hospice/palliative care experience preferred.
State social worker license in the employing state is required.

Responsibilities

Provides psychosocial evaluations, ongoing counseling and bereavement services to terminally ill patients and families.
Assesses psychosocial needs and creates a social work plan of care with the interdisciplinary team.
Evaluates hospice level of care and facilitates transitions between care levels.
