Upstate Caring Partners seeks a Service Coordinator for Early Intervention to build and sustain services for families with children birth to 3 years with developmental needs. The role involves coordinating meetings, referrals, advocacy, and education for families and providers.

Responsibilities include establishing and monitoring services, collaborating with providers, maintaining documentation, and advocating for additional supports.

Competenze

  • Two years’ experience in service coordination activities or equivalent
  • One year of service coordination plus experience in a setting with infants/toddlers with delays or disabilities
  • One year of service coordination experience and an Associate’s degree or a Bachelor’s degree in health/human service fields
  • A license, certification, or registration in a listed profession
  • Must have or obtain NYS EI Coordination Certification within 3 months
  • Must complete 40 hours of specialized training within the first year
  • Valid NYS driver’s license

The Early Intervention Service Coordinator establishes relationships with families of children aged birth to 3 years with developmental disabilities in order to create and sustain services identified through the development of an Individual Family Service Plan. This Service Coordinator schedules and conducts meetings, makes referrals, and provides advocacy and education for children and their families.

Core Responsibilities

  • Provides initial and/or ongoing service coordination across Oneida County.
  • Ensures all services are established and maintained for the families.
  • Assures supports are in place inclusive of service provider and family contacts.
  • Collaborates with all service providers and establishes team communication plan.
  • Monitors goals on a continuing basis and quality of team interactions.
  • Advocates for additional services and linkages as appropriate.
  • Provides written documentation of services.

Qualifications

  • A minimum of one of the following educational or service coordination experience credentials:
    • two years’ experience in service coordination activities as delineated in this subpart (voluntary or part‑time experience which can be verified will be accepted on a pro rata basis); or,
    • one year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities; or,
    • one year of service coordination experience and an Associate’s degree in a health or human service field; or
    • a bachelor’s degree in a health or human service field; or
    • a license, certification, or registration in one of the professions listed in section 69‑4.1(al) of this subpart
  • Must have or obtain the NYS Early Intervention Coordination Certification within the first 3-months in this position.
  • Must complete 40 hours of specialized/designated training within the first year in this role per regulatory requirements.
  • Valid NYS driver's license required.
