Description

ExcelHealth Group is growing, and we are looking for a motivated traveling Optometrist who wants to do meaningful work beyond the walls of a traditional clinic. This is an exciting opportunity for a clinician who values autonomy, variety, purpose, and strong operational support. You will have the chance to build relationships with facility teams, serve a deeply appreciative patient population, and be part of a care model designed to meet people where they are.

Why Join ExcelHealth Group?

  • Practice with independence while still having a responsive support team handling logistics, facility coordination, scheduling, billing, collections, recruitment, and technology needs.
  • Enjoy variety in your workweek through a mobile care model that allows you to serve multiple communities and meet patients in different care settings.
  • Join a mission-driven organization focused on compassionate, comprehensive care for residents in nursing homes and long-term care communities.
  • Be part of a team that values professionalism, reliability, clinical quality, and the human connection that makes healthcare meaningful.
About ExcelHealth Group

ExcelHealth Group provides mobile and ancillary healthcare services for residents in long-term care communities, including optometry, dentistry, podiatry, and audiology. We partner with nursing facilities to make care easier to access by bringing qualified providers directly to residents where they live.

Our model is built around access, compassion, and operational excellence. ExcelHealth Group supports clinicians by managing the non-clinical side of the work—including facility contracting, relationship management, growth and development, patient enrollment, scheduling, billing, collections, recruitment, and IT support—so providers can focus on what they do best: delivering excellent patient care.

As we continue to grow, we are looking for clinicians who are energized by service, comfortable working independently, and excited to help expand access to high-quality care for an underserved population.

Where You’ll Work
  • Provide mobile optometry services at nursing homes and long-term care communities throughout the state.
  • Travel to established partner facilities where residents are scheduled for on-site care.
  • Work in designated treatment rooms or provide bedside care when needed based on patient needs and facility setup.
  • Receive schedules in advance to support planning, organization, and predictability in your workweek.
What You’ll Do
  • Conduct comprehensive eye examinations for residents in long-term care settings.
  • Provide yearly exams, follow-up visits, spectacle ordering, and evaluation or management of common eye disorders.

  • Document clinical care accurately and thoroughly in accordance with company and regulatory expectations.
  • Communicate professionally with patients, facility staff, families when appropriate, and internal team members.
  • Represent ExcelHealth Group with compassion, reliability, and a strong commitment to patient-centered care.
What We’re Looking For
  • Licensed Optometrist with a passion for delivering high-quality, compassionate clinical care.
  • Medicaid provider number, or willingness to obtain one promptly.
  • A patient-first mindset and genuine interest in serving seniors and long-term care residents.
  • Strong communication skills and the ability to build positive relationships with patients, facility staff, and internal team members.
  • Comfort working independently, managing a mobile schedule, and adapting to different facility environments.
  • Professionalism, dependability, flexibility, and a desire to be part of a growing care model.
