Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with the primary care physician (PCP) on patient education and follow‑up, as necessary.
• Provide services at the member’s place of residence, which can be in their homes or skilled nursing facilities.

• Flexibility – Visits are scheduled based on your availability, from Sunday – Saturday 8 am – 7 pm.
• Competitive Pay – Earn income based on the number of assessments completed. Those completing 20‑100 visits/month earn $1,800‑$11,500. Pay ranges $90‑$115 per assessment.

Qualifications

Skills & Experience That Shine

• Master’s degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice, including prescription authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS, or CPR certification.
• 1 year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license and proof of adequate automobile insurance.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
