Medicasa Italia seeks a nurse to support home-based patient care, including installation of medical devices and training for families. You will coordinate with the multidisciplinary team to ensure care plans are followed in accordance with prescriptions and local regulations.

The role emphasizes patient education, proactive monitoring of clinical outcomes, and collaboration with health coordinators to maintain high-quality home care services.

Competenze

• Diploma in Nursing with at least 5 years of experience.
• Competence in installing medical devices.
• Excellent communication and relational skills with patients.

Mansioni

• Act as the primary point of reference for the patient, family (caregiver) and the multidisciplinary team within the Central Operational Office.
• Facilitate and optimize relationships and communication with General Practitioners, Health Districts and territorial operators.
• Monitor the correct application of Individual Care Plans, ensuring clinical appropriateness and alignment with predetermined care objectives, in collaboration with the Health Coordinator.
• Assign care activities to operators, handle clinical‑health communications and interface with ATS applications.
• Ensure continuous and proactive monitoring of clinical outcomes and health data using the electronic clinical chart and available digital tools, managing the preparation and verification of documentation and periodic reports.
• Support operational management (e.g., logistics of medical materials, archiving), contribute to onboarding, training and education of new healthcare operators in the area, and guarantee correct use of medical devices provided by Medicasa.

Role Overview
