Responsibilities

• Assist physicians in patient assessment and clinical documentation.
• Review patient medical histories, medication profiles, laboratory reports, and treatment plans.
• Support medication reconciliation and identify potential drug interactions, duplications, allergies, and medication-related problems.
• Provide appropriate medication-related information to physicians and the clinical team.
• Monitor patients for medication-related adverse effects and therapeutic outcomes.
• Assist in preparing and maintaining patient case sheets and clinical records.
• Coordinate with doctors, nurses, pharmacists, laboratories, and other healthcare professionals.
• Participate in ward rounds and clinical discussions as required.
• Follow up on investigations, reports, and physician instructions.
• Educate patients regarding medications, adherence, precautions, and lifestyle-related aspects as directed by the treating physician.
• Ensure accurate and timely clinical documentation.
• Maintain patient confidentiality and comply with hospital policies and SOPs.
• Support quality improvement, clinical audits, and patient-safety initiatives.
