Role Description

The Registered Nurse Coordinator is a full-time, on-site role based in Sahibzada Ajit Singh Nagar, responsible for overseeing and coordinating patient care services in the home health setting. Day-to-day activities include assessing patient needs, developing and updating care plans, administering nursing treatments, and monitoring patient progress. The role involves supervising and guiding other care team members, including therapists and support staff, to ensure care is delivered according to clinical standards and regulatory requirements. The Registered Nurse Coordinator will communicate regularly with patients, families, physicians, and other healthcare providers to facilitate continuity of care, provide education on treatment plans, and address questions or concerns. Documentation of assessments, interventions, and outcomes in patient records and participation in quality improvement and safety initiatives are also key responsibilities.

Qualifications
