Job summary

Elara Caring is seeking a Physical Therapist to join our home-based care team. You will evaluate and treat patients, develop individualized plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

The role requires travel up to 50% and involves providing high-quality care in patients' homes while maintaining thorough documentation and adhering to clinical guidelines.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Minimum of one year of experience in a healthcare setting.
• Willingness and ability to travel up to 50% within the assigned coverage area.
• Dependable vehicle, valid driver’s license, and current auto insurance.

Responsibilities

• Provide direct patient care, including evaluations, treatment planning, and outcome assessments.
• Design and implement individualized plans of care tailored to each patient’s needs.
• Collaborate with interdisciplinary team members to ensure optimal outcomes.
• Direct and support therapy assistants and staff to achieve patient goals.
• Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders.

What You’ll Do

• Promote Elara Caring’s mission, philosophy, and policies to ensure exceptional patient care

• Provide direct patient care, including evaluations, treatment planning, and outcome assessments

• Design and implement individualized plans of care tailored to each patient’s needs

• Collaborate with interdisciplinary team members to ensure optimal outcomes

• Direct and support therapy assistants and staff to achieve patient goals

• Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders

• Serve as a clinical resource and promote physical therapy within the community

• Work in a physically demanding, fast-paced home-care environment

• Perform lifting, transferring, and positioning of patients as required

• May work irregular hours or participate in call rotation, if applicable

What We’re Looking For

• Graduate of an APTA-accredited Physical Therapy program

• Current, unrestricted Physical Therapist license in the state of practice

• Minimum of one (1) year of experience in a healthcare setting

• Willingness and ability to travel up to 50% within the assigned coverage area

• Dependable vehicle, valid driver’s license, and current auto insurance

• Ability to lift, push, pull, and support up to 50 pounds

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee .
