Indian Spinal Injuries Centre is seeking a dedicated Medical Transcriptionist to transcribe and prepare patient discharge summaries from doctors' notes, dictations, and medical records. You will verify demographics, ensure accuracy of diagnoses, investigations, medications, and follow-up instructions, and coordinate with doctors, nursing staff, and other clinical teams to resolve discrepancies.

Competenze

• Transcribe and prepare patient discharge summaries from doctors notes and medical records.
• Ensure accuracy of medical terminology, diagnoses, investigations, treatment details, medications, and follow-up instructions.
• Coordinate with doctors, nursing staff, and other clinical teams to obtain missing information and clarify discrepancies.
• Ensure discharge summaries are completed within the prescribed turnaround time (TAT).
• Proofread and edit medical documents for grammatical accuracy, consistency, and completeness.
• Maintain patient confidentiality and comply with hospital policies and applicable data protection requirements.
• Verify patient demographics, admission details, diagnosis, and discharge information before finalizing documents.
• Follow up with concerned departments for pending discharge summaries and documentation.
• Maintain proper records of discharge documentation and ensure timely updates in the Hospital Information System (HIS).
• Support the discharge process by ensuring documentation is accurate, complete, and available on time.
