Job summary

JobItUs is recruiting a Medical Coder in Vadodara, India to review and audit clinical documentation, assign CPT/ICD-10-CM/HCPCS codes, and ensure coding accuracy and regulatory compliance.

The role involves ongoing audits, E/M code reviews, and communicating coding discrepancies to cross-functional teams, while maintaining patient confidentiality per HIPAA and keeping current with coding guidelines and industry updates.

Qualifications

• Review and audit clinical documentation and medical notes for accuracy and completeness.
• Assign CPT, ICD-10-CM, and HCPCS codes based on provider documentation.
• Ensure accurate coding in accordance with official coding guidelines and payer requirements.

Responsibilities

• Identify coding errors, discrepancies, and missing documentation.
• Verify that medical notes support the assigned codes and billed services.
• Perform regular audits to ensure coding accuracy and compliance.
• Review E/M codes and ensure appropriate level selection.
