Job summary

Acumen 360 Consulting & HR Solutions is seeking a medical coder in Vadodara to review clinical documentation, assign CPT/ICD-10-CM/HCPCS codes, and ensure compliance with guidelines and payer requirements.

You will perform regular audits, verify documentation supports codes billed, and communicate discrepancies to relevant teams while maintaining patient confidentiality under HIPAA guidelines and staying current with CPT updates and industry changes.

Responsibilities

• Review and audit clinical documentation and medical notes for accuracy and completeness.
• Assign CPT, ICD-10-CM, and HCPCS codes based on provider documentation.
• Ensure coding aligns with official guidelines and payer requirements.
• Identify coding errors, discrepancies, and missing documentation.
• Verify notes support the assigned codes and billed services.
• Perform regular audits to ensure coding accuracy and compliance.
• Review E/M codes and ensure appropriate level selection.
• Communicate coding discrepancies and provide feedback to relevant teams.
• Maintain confidentiality of patient information in compliance with HIPAA regulations.
• Stay updated on CPT changes, coding guidelines, and industry updates.
