CommuniCare Rehab is seeking a Physical Therapist (PT) at Southwood Healthcare Center in Terre Haute, IN. This full-time role offers a $10,000 sign-on bonus to join our close-knit, mission-driven network of 140 centers.

You will evaluate charts, develop and implement treatment plans, supervise PTAs, and participate in interdisciplinary meetings to enhance resident care across multiple centers.

Qualifikationen

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Active state Physical Therapy License (valid or pending).

Qualifications:

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
