• Map communities linked to each project school and conduct community awareness meetings.
• Identify diabetes ambassadors to sustain awareness beyond the project.
• Documentation and reporting: compile data for submission to District Coordinator / Programme Manager.
• Provide field data and inputs for programme reports in coordination with the ME Officer.
• Maintain complete records of referrals to health facilities, forms, and related documentation.
• Travel: approximately 20 days of field travel per month; travel and logistics support provided.
