Job summary

Seniors Hub in Bengaluru, a technology-enabled elder care platform, seeks a Care Manager to be the single point of contact for 30 to 40 senior families, coordinating healthcare, emergencies, daily support, and engagement to ensure safety and well-being.

You will arrange doctor visits, diagnostics, nursing and physiotherapy, monitor vitals and medication adherence, respond to emergencies, and supervise concierge services and home visits.

Qualifications

• Graduate with preference for BSc Nursing, Social Work or healthcare diploma.
• 2–5 years of elder care, healthcare coordination or related experience.
• Excellent communication, empathy, and calm decision making under pressure.
• Willingness to travel across Bangalore; own vehicle advantageous.

Responsibilities

• Own end to end care delivery for 30–40 senior families.
• Coordinate doctor visits, diagnostics, nursing, and physiotherapy; track vitals and meds.
• Act as first responder during emergencies; coordinate ambulance and hospitalization.
• Coordinate concierge services via vendors for payments, transport, and home maintenance.
• Conduct regular home visits to check seniors' wellbeing.
• Keep documentation accurate and share updates with families.

About the Role:
Seniors Hub is a technology enabled elder care platform that gives families a trusted local guardian for their aging parents. As a Care Manager, you will be the single point of contact for 30 to 40 senior families, coordinating healthcare, emergencies, daily support, and engagement to ensure their safety and well being.

Key Responsibilities:

• Own end to end care delivery for assigned families, covering health, emergency, concierge, and engagement needs
• Coordinate doctor visits, diagnostics, nursing, and physiotherapy, and track vitals and medication adherence
• Act as first responder during emergencies, coordinating ambulance, hospitalization, and keeping families updated
• Coordinate concierge services like bill payments, transport, and home maintenance through vendors
• Conduct regular home visits to check on seniors' physical and emotional wellbeing
• Keep documentation accurate and up to date, and share regular updates with families

Requirements:

• Any graduate; BSc Nursing, Social Work, or a healthcare/care giving diploma preferred
• 2 to 5 years of experience in elder care, healthcare coordination, nursing, or community health
• Good communication skills, empathy, and the ability to stay calm and make good decisions under pressure
• Willing to travel extensively across Bangalore
• Candidates having own vehicle for mobility will be an added advantage
