Job summary

Bayside Health is seeking an experienced Transition Care Program Case Manager to coordinate a comprehensive package of care for older adults transitioning from hospital to home or other supportive care settings. You will work within a multidisciplinary team to ensure timely, safe, and person-centred discharge planning and ongoing support.

Qualifications

• Registration as a Registered Nurse with the NMBA via AHPRA; or Registration as an Allied Health Practitioner with AHPRA.
• Minimum 5 years of clinical experience.
• Current Victorian Driver's Licence.
• Current and satisfactory Police Check.
• International Police Check if you have worked outside Australia within the last 10 years.
• Postgraduate qualification, or progress towards one.

Responsibilities

• Coordinating admissions into the Transition Care Program.
• Completing assessments and developing individualised Care and Service Plans with clients.
• Coordinating community-based services and supports to meet client goals.
• Monitoring client progress and supporting achievement of reablement outcomes.
• Facilitating discharge planning and transitions to long-term support arrangements.
• Supporting clinical care coordination and client management where required.
• Liaising with hospitals, clinics, GPs and community providers to ensure seamless care delivery.
