Job summary

Medforce is seeking an Occupational Therapist to join our team supporting older adults across assessment, rehabilitation, and safe discharge planning. You will contribute to high-quality patient flow as part of a dynamic multidisciplinary team and lead on complex cases to promote independence.

The role requires significant post-qualification experience with older adults in various settings, holistic assessments, and expertise in discharge planning, safeguarding, and home adaptations.

Qualifications

• Significant post-qualification experience working with older adults in acute, community, or intermediate care settings.
• Demonstrated ability to complete holistic and functional assessments with older adults, including frailty, cognitive impairment, or multi-morbidity.
• Experience delivering patient-centred rehabilitation programmes to maximise independence and reduce hospital length of stay.
• Proven competence in managing complex discharge planning, including coordination with MDTs and external agencies.
• Ability to assess for, recommend, and provide assistive equipment and minor home adaptations to support safety and independence.

Responsibilities

• Assess and rehabilitate older adults to promote independence and functional wellbeing.
• Lead on complex cases within a multidisciplinary team.
• Coordinate safe discharge planning with external agencies and teams.
• Provide education to patients, families, and staff on discharge and home management strategies.
• Write clear, concise clinical documentation and reports.
• Prioritise workload and manage time effectively in fast-paced settings.
