Job summary

Lehigh Valley Health Network in Pittston seeks a dedicated Physical Therapist to evaluate, plan, direct and administer physical therapy programs to restore function and help patients reach maximum independence. You will assess patient condition based on physician diagnosis and collaborate with the patient and family to develop a care plan.

Provides therapy using exercises, manual techniques, functional mobility training, activities of daily living training, and patient education.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Current PA Physical Therapy license upon hire.
• American Heart Association BLS required.
• Ability to travel to patient care assignments - Home Care only.
• Strong communication with patients and team members.

Responsibilities

• Establish and implement PT plans to restore function and independence.
• Deliver evidence-based therapy: exercises, manual techniques, mobility training, ADL training.
• Assess patient progress and modify plans as needed.
• Communicate patient progress to supervisors and physicians.
• Provide care per regulatory requirements and billing guidelines.
• Document all care activities and outcomes accurately.
