Job summary

Dublin Simon Community is seeking a Tissue Viability Nurse to provide specialist clinical expertise across a 100-bed unit at Ushers Island HACF. You will lead wound care for residents with complex needs, including addiction, chronic illness, and homelessness, and work closely with the Primary Care Tissue Viability Service to ensure integrated care.

Qualifications

Registered Nurse on the NMBI active register.
Postgraduate qualification in Tissue Viability, Wound Management or closely related area (NFQ Level 9+).
Minimum of 3 years' post‑registration nursing experience with tissue viability/wound management.
Experience at Clinical Nurse Specialist level with autonomous practice, leadership, audit, and education.
Experience in health and addiction care settings or with vulnerable populations.

Responsibilities

About the Role

The Tissue Viability Nurse will provide specialist clinical expertise, leadership, and support in the prevention, assessment, and management of wounds for residents of Ushers Island HACF. This role is essential in supporting individuals with complex needs, including addiction, chronic health conditions, homelessness, and unstable health literacy.

The post holder will work across the 100-bed unit with a main focus on the Step Up, Step Down Surgical Service, delivering person-centred, trauma-informed care, and ensuring best practice in tissue viability. This role includes close partnership working with the Primary Care Tissue Viability Service, ensuring continuity of care, shared learning, and integrated practice.

Contract Type: full-time, permanent

Work Hours: 37.5 hours, 5-day liability

Reports to: Assistant Director of Nursing

Role Responsibilities

Clinical Practice

Provide expert assessment, diagnosis, treatment planning, and management of acute and chronic wounds, pressure injuries, skin integrity concerns, leg ulcers, diabetic foot wounds, traumatic wounds, and complex non-healing wound presentations.
Lead on the development and implementation of wound care plans for residents with multifaceted physical, social, and addiction-related needs.
Conduct regular wound audits, risk assessments (including Waterlow/Braden), and reviews of clinical practice.
Support nursing and support staff in timely identification and escalation of deteriorating wounds or skin integrity issues.
Act as a clinical resource to the multidisciplinary team, offering specialist advice and bedside teaching.

Education and Training

Deliver training and competency development to nurses, HCAs, and MDT members on wound assessment, pressure injury prevention, dressing selection, and documentation.
Promote trauma-informed approaches when working with people who may have anxieties around care, bodily exposure, or previous negative healthcare experiences.
Provide ongoing coaching and reflective learning to enhance practice across the unit.

Governance & Quality Improvement

Contribute to the development and review of clinical policies, procedures, and guidelines relating to tissue viability.
Monitor compliance with national standards, HIQA requirements, and internal governance systems.
Lead or participate in quality improvement initiatives, audits, and PDSA cycles aimed at improving skin integrity outcomes.
Monitor and report on wound prevalence, pressure injury incidence, tissue viability trends, and relevant quality indicators to support organisational learning and service improvement.
Support the standardisation and appropriate use of wound care products to ensure safe, effective, evidence-based and cost-conscious practice.
Maintain accurate, timely, and high-quality clinical documentation.
Work in partnership with the Primary Care Tissue Viability Nurse to coordinate joint assessments, align care plans, and facilitate continuity of care for residents transitioning into or out of the service.
Liaise with external acute hospitals, community health teams, and addiction services where required.
Facilitate timely referral to appropriate external services, including GP, vascular, podiatry, dermatology, infectious diseases, acute hospital and community services, where clinically indicated.
Contribute to case reviews, MDT meetings, and discharge planning processes.

Professional Responsibilities

Maintain professional registration with the Nursing and Midwifery Board of Ireland (NMBI).
Uphold the NMBI Code of Professional Conduct and Ethics.
Maintain up-to-date clinical knowledge, including emerging evidence in wound care, dressing technologies, and pressure injury prevention.
Critically appraise current evidence and research in tissue viability and wound management, translating relevant findings into clinical practice, guidelines, education and quality improvement activity.
Participate in supervision, CPD, and reflective practice sessions as required.

Qualifications & Experience

Registered Nurse on the active register maintained by the Nursing and Midwifery Board of Ireland (NMBI), registered in the appropriate division of the register for the post.
Relevant post-registration/postgraduate qualification in Tissue Viability, Wound Management or a closely related specialist area, preferably at NFQ Level 9 or above; minimum Level 8 qualification may be considered where supported by substantial specialist experience and an agreed professional development pathway.
Minimum of 3 years’ post-registration nursing experience, including demonstrable specialist experience in tissue viability/wound management and the assessment and management of complex wounds, pressure injury prevention, leg ulcer care, diabetic foot wounds and non-healing wounds.
Evidence of working at Clinical Nurse Specialist level, including autonomous specialist practice, clinical leadership, education, audit, quality improvement, policy development and service improvement.
Demonstrate depth and breadth of nursing experience in the specialist area of wound management and tissue viability relevant to the role.
Commitment to undertake any additional CNS-related professional development, including relevant prescribing or specialist practice education, where required by organisational policy, service need or national guidance.
Experience working in settings involving addiction, homelessness, or vulnerable populations (or a strong understanding of related issues).
Experience in health and addiction care settings.
Training in trauma-informed care or psychological safety.
Experience delivering staff training or quality improvement initiatives.
Advanced clinical assessment and wound management skills.
Ability to work autonomously while contributing to a collaborative culture.
Excellent communication and interpersonal skills, with a strengths-based, non-judgmental approach.
Strong organisational, audit, and report-writing skills.
Ability to support staff skill development and clinical decision-making.
