Qualifications

• Qualification in Healthcare, Social Care or related discipline.
• Excellent spoken and written English for daily communication.
• Understanding of safeguarding and person-centred practice.
• Experience working in disability or social care and holding a driving licence.

Responsibilities

• Deliver high-quality person-centred support to individuals in residential and community settings.
• Act as a Key Worker for allocated individuals and monitor goals.
• Support daily living tasks while maintaining dignity and respect.
• Administer medication in line with policy and procedures.
• Assist individuals to access their community and engage in activities.
• Support daily household activities and education, employment, leisure goals.
• Collaborate with families and multidisciplinary teams to ensure continuity of care.
• Promote safeguarding and positive outcomes in all interactions.
