Job summary

• Social Care Worker supports people to achieve their vision and goals as per their personal plans.
• Empowers people to live self-determined lives in socially inclusive communities.
• Supports families as appropriate and in line with the person’s wishes.
• Builds social capital in communities and helps people participate in valued roles.
• Collaborates with support, leadership, clinical teams and other stakeholders to ensure safe, effective services and HIQA compliance.
