Job summary

Ontario Shores Centre for Mental Health Sciences in Whitby, Ontario, seeks a Registered Practical Nurse for the Assertive Community Treatment Team (ACTT). The successful candidate will assess mental health and physical health needs and work within a multidisciplinary team.

You will provide client-centred treatment and rehabilitation, develop goals with clients and families, and deliver psychosocial interventions, medication support, and crisis intervention as needed.

Qualifications

• Registration with the College of Nurses of Ontario in good standing.
• Current CPR – Health Care Provider (HCP) certification.
• Demonstrated knowledge of CNO Standards of Practice and reflective practice.
• Knowledge of Canadian Standards for Psychiatric – Mental Health Nursing and related policies.
• Experience with multis-disciplinary teams in mental health settings.
• Experience in assertive community treatment and psychosocial rehabilitation.

Responsibilities

• Provide mental health and physical assessments within a multidisciplinary team.
• Deliver assertive community treatment to clients with severe mental illness.
• Develop treatment and rehabilitation goals with clients and families.
• Provide clinical case management, psychosocial interventions and support.
• Assist with activities of daily living, harm reduction, and vocational rehab.
• Crisis intervention and coordination with emergency services as needed.
