Job summary

Ontario Shores Centre for Mental Health Sciences in Whitby, Ontario is seeking a Registered Nurse 3 for the Durham ACT team. The successful candidate will perform mental health and physical assessments and function within a multidisciplinary team to deliver assertive community treatment.

Responsibilities include client-centered rehabilitation, treatment planning with clients and families, and ongoing case management alongside medication administration and crisis intervention as needed.

Qualifications

• Certificate of Competence from the College of Nurses of Ontario is required; BScN preferred.
• Current CPR - Health Care Provider (HCP) is required.
• Demonstrated knowledge of Psychosocial Rehabilitation and Recovery.
• 3–5 years psychiatric experience desirable; community mental health experience preferred.
• Familiarity with Mental Health Act and PHIPA is required.

Responsibilities

• Provide mental health and physical assessments and participate as a multidisciplinary team member.
• Deliver assertive community treatment to clients with severe mental illness.
• Develop treatment and rehabilitation goals with clients and families.
• Provide clinical case management, psychosocial interventions, and medication monitoring.
• Assist clients in activities of daily living and harm-reduction activities.
• Monitor for deterioration and intervene to prevent hospitalization.
• Provide emergency crisis intervention as needed.
