Job summary

The Rehab Group in Stillorgan, Co Dublin, is seeking a Programme Facilitator. You will coordinate person-centred programmes, support service users with independent living skills, and help with community integration in a 39-hour-per-week role.

The role requires QQI Level 5 in Social Care or equivalent, with a third-level health/social care background desirable. A full driving licence is helpful. Salary starts at €16.38–€17.23 per hour with progression up to €17.71, depending on experience.

  • A third level graduate in Health & Social Care or equivalent is desirable.
  • Experience supporting adults with an intellectual disability and people with mental health issues.
  • 1 year minimum experience in a relevant setting with person centred plans and programme development.

Responsibilities

  • Be a key worker to facilitate the Person Centred Planning process and maintain required records.
  • Develop and deliver person centred programmes to meet identified needs and maintain records.
  • Support individuals and groups in accessing their community by facilitating activities and other duties as deemed appropriate.

About The Role

Reporting to the Community Service Manager through the Programmes Supervisor, the post holder will facilitate service users in their choices, developing programmes within the context of a person centred model of service delivery. The programmes may include independent living, personal development and occupational and leisure activities, within the context of a quality of life model.

What We’re Looking For
  • Minimum QQI Level 5 in Social Care or equivalent
  • A third level graduate, Health & Social Care or equivalent is desirable.
  • Experience supporting adults with an intellectual disability and people enduring mental health issues
  • 1 year minimum experience of working in a relevant Settingwith the experience of facilitating and implementing person centred plans, experience in the design, development and provision of programmes and activities to meet the needs, choices and abilities of service users including providing support in the area of independent living skills and community integration for persons with intellectual disability.

Your Responsibilities
  • To be a key worker to service users in order to facilitate the Person Centred Planning process. To identify goals and supports required by individual service users and maintain records as required.
  • To develop and deliver person centred programmes to meet the needs of service users as identified through Person Centred Planning and to maintain the required records.
  • To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.
Additional Requirements
  • A full, clean driving licence, with a minimum of 2 years' experience is desirable.

If applicable, candidates will need to provide a police certificate for any country they've lived in before moving to Ireland

We offer a competitive starting salary of

€16.38– €17.23 per hour (Points 1–5 of the scale), based on qualifications and experience. The 12-point salary scale provides annual progression opportunities up to €17.71 per hour.
