Job summary

The Rehab Group in Ireland is seeking a Programmes Facilitator on a fixed term, 39-hour week contract to promote independent living and community integration for service users with intellectual, physical, sensory, or mental health challenges.

Role involves facilitating educational, developmental, recreational and occupational programmes, tailored to individual needs and abilities, with emphasis on supported living and community engagement.

Qualifications

• Minimum QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Additional qualification/experience in supporting adults with Autism would be desirable.

Responsibilities

• Promote and support independent living and community integration for individuals with disabilities.
• Facilitate person-centred programmes and activities to meet service users' needs and abilities.
• Develop and deliver person-centred plans, maintain required records, and support independent living skills.

Fixed Term Variable hour Contract, 39 hours per week

Location:Grand Canal House ,Tullamore, Co Offaly

About the Role:

To promote and support independent living and community integrationofindividuals with intellectual, physical, sensory,and mental health challenges.Supporting individuals to be valued members of their local communities. Tofacilitatein the educational, developmental,recreationaland occupational programmes and activities of the service, asappropriate. Programmes will be designed to match the service user’s needs and capabilities while allowing him/her to develop his/her full potential and will be implemented atan appropriate timeand settingin order togive maximum support.

What We’re Looking For:

• Minimum QQI Level 5 in Social Care or equivalent
• A third level graduate, Health & Social Care or equivalent is desirable.
• Additionalqualification/experiencein supporting adults with Autismwould be desirable.
• Willingness to attend conferences / training courses when necessary
• 1 year minimum experience of working in a relevant Settingwith the experience of facilitating and implementing personcentredplans, experience in the design, development and provision ofprogrammesand activities to meet the needs, choices and abilities of service users including providing support in the area of independent living skills and community integration for persons with intellectual disability.

Your Responsibilities:

• To be a key worker to service usersin order tofacilitate thePerson CentredPlanning process. Toidentifygoals and supports required by individual service users andmaintainrecords asrequired.
• To develop and deliver person centred programmes to meet the needs ofservice users asidentifiedthrough Person Centred Planning and tomaintainthe required records.

Additional Requirements:

• A full, clean driving licence, with a minimum of 2 years’ experience is desirable.

If applicable, candidates will need to provide a police certificate for any country they've lived in before moving to Ireland

We offer a competitive starting salary of€16.38– €17.23per hour(Points 1–5 of the scale), based on qualifications and experience. The 12-point salary scale provides annual progression opportunities up to€17.71 per hour.
