Allied Health/Nursing seeks an Occupational Therapist to provide person-centred assessment, intervention, rehabilitation and health promotion within a primary care setting. Open to Staff or Senior OTs, the role focuses on preventing hospital admissions and promoting self-management and independence.

You will work within a multidisciplinary team including GPs, physiotherapists, S&L therapists, public health nurses and social workers, contributing to integrated care pathways and CPD.

Competenze

• Degree in Occupational Therapy and CORU registered.
• Experience in community or primary care settings.
• Ability to work in multidisciplinary teams and manage a varied caseload.

Mansioni

• Conduct comprehensive occupational therapy assessments in clinic, home, community, or virtual settings.
• Develop and implement individualised intervention plans based on client goals.
• Assess ADLs, mobility, cognition, sensory processing and environmental factors.
• Provide rehabilitation and reablement to maximise independence.
• Deliver falls prevention, energy conservation, and self-management programmes.
• Maintain accurate, timely, and legally compliant clinical documentation.
• Participate in discharge planning and onward referrals as required.
