Job summary

Caliber Therapies in Waterford, WI is seeking a dedicated provider to join our team on a contract basis. The role includes driving to client homes, assisting with transfers, and supporting activities of daily living, with mileage reimbursement.

Ideal candidates hold a Bachelor’s degree and have experience with brain injury, IDD, mental health, or veterans. Tablet for documentation (iPad) is preferred or required, and reliable transportation plus a clean driving history are expected.

Qualifications

• Bachelor’s degree from an accredited university.
• Experience with brain injury, IDD, mental health, and/or veteran populations.
• Military service or veteran experience is a plus.
• Ownership of or willingness to purchase a tablet for documentation.

Responsibilities

• Drive to client locations as needed and assist with transfers.
• Support activities of daily living (ADLs) for clients.
• Document care activities using a tablet.
• Ensure safety and professionalism during visits.

Position Details:

• Hourly rate: $20/hr
• Service Area: Waterford, WI (mileage reimbursed)
• Employment Type: Contract position with potential for increased hours based on performance and availability
• Responsibilities: May include driving, assisting with transfers, and supporting activities of daily living (ADLs) as needed
