Praxis Care is seeking an experienced Relief Activity Facilitator in Mulhuddart, Ireland. You will work as part of a dedicated team to deliver high-quality care and support, fostering independence and dignity for people we support.

The role involves promoting safety, collaboration with team leaders, and adherence to policies and procedures in a respectful environment. We value flexible, community‑based support with a focus on person-centred plans, life skills development and practical assistance

Qualifications

• Full valid ROI/UK/EU driving licence and access to own vehicle.
• Strong risk assessment, leadership and problem-solving skills.
• Excellent organisational, time management, numeracy and literacy abilities.
• Effective communication and teamwork to support diverse clients.

Responsabilités

• Support and empower individuals to develop independent living skills as identified in person-centred plans.
• Collaborate with Team Leaders and managers to prepare and implement individual and group support programmes.
• Promote a safe working and living environment and resolve conflicts or challenging behaviours using appropriate techniques.
• Ensure medication is stored and administered in line with Praxis Care’s Medication Policy where required.
• Record and log relevant information per service reporting guidelines.
• Stay informed about all policies and procedures and participate in related training.
