Job summary

Dublin Simon Community in Dublin City Centre is seeking an Intensive Case Manager to engage clients referred to Housing First and deliver holistic support for complex health and addiction needs.

You will build trusting relationships, promote tenancy sustainment, and coordinate care with health and social services, while applying harm reduction and person-centered practices.

Qualifications

• 2 years’ experience in a related social care setting, working with vulnerable clients OR 1 year as a project worker (case management)
• BA/BS degree in psychology, social care, addiction, counselling, youth work, community development, psychotherapy or similar
• Understanding of homelessness and client needs
• Ability to support clients independently within a team
• Strong relationships with clients and stakeholders

Responsibilities

• Proactively engage and support clients referred to Housing First
• Build and maintain trusting, professional relationships with clients to promote sustained engagement
• Provide ongoing support to clients with complex physical health, mental health, and substance misuse needs
• Support clients in sustaining tenancy and escalate issues to the team leader
• Apply a harm reduction approach when working with clients experiencing addiction or high‑risk behaviours
• Assess client needs and tailor support plans
• Maintain up-to-date knowledge of local health and support services
• Make timely and appropriate referrals to health and support services
• Maintain accurate support plans, case notes and risk assessments
• Safeguard vulnerable adults and children per policies
• Collaborate with partner agencies for coordinated support
• Build relationships with local councils and health services
• Drive and implement care and case management processes while lone working
• Attend weekly meetings and planning days
• Advocate for clients
• Assist clients with mediation, social welfare, and other supports
• Record caseload data and update PASS and IT systems
• Assist Non-English speaking clients with translation services

About The Role

The Intensive Case Manager Team will proactively engage all clients that are referred to Housing First. The team will use their qualifications and experience to continue to build relationships to support clients with ongoing physical and mental health needs along with addiction support needs. The team will be trained in a harm reduction approach and have a thorough knowledge of the health services available to the clients we support. This will include making referrals to the health service and safeguarding vulnerable Adults and children.

Contract Type: full-time, permanent (hiring for 8 roles)

Working Hours: 37.5 hours, 7-day liability (includes mornings, days, evenings, and weekends)

Location: Dublin City Centre

Salary: 38,000 – 43,750 DOE

Role Responsibilities

• Use professional qualifications and experience to assess client needs and tailor appropriate support plans
• Maintain up-to-date knowledge of local health and support services available to clients
• Make timely and appropriate referrals to relevant health and support services.
• Maintain accurate and current support plan, case notes and risk assessments on PASS and organisational databases in accordance with service level agreement.
• Safeguard vulnerable adults and children in line with organisational policies and statutory requirements
• Work collaboratively with partner agencies to ensure coordinated and effective client support
• Build and maintain professional relationships with local councils and health care services
• Drive and implement care and case management processes and protocols whilst lone working in client’s home. Carry out risk assessments and follow safeguarding protocols to vulnerable clients and children.
• Attend weekly team meetings, planning days, and function planning days
• Advocate on behalf of clients
• Facilitate clients to access mediation services, dispute resolution or anger management training, social welfare payments, other relevant financial supports, training, education, and employment schemes/ programmes, Citizens Information, Free legal Aid services, health and care facilities (GP, Mental and physical health services health, Family support services, Counselling/psychology services (clinical), Childcare and community based education.
• Record client caseload data and update regularly. Compile statistics, update PASS and Internal IT Systems.
• Support Non-English speaking clients to access the Dublin Simon Translation Service, ensuring that all clients have the clear and correct information, and can be understood.

Qualifications and Experience

• Understanding of why people become homeless and the needs they have.
• Understanding of supporting clients to settle into their new tenancies.

• Experience developing strong relationships, with clients and stakeholders.
• Ability to support clients independently, within the support of a team.

Desirable

• Full driver’s license.
• Experience of working in the community, with child/family support, and liaising with local authorities and/or other housing providers.

• Awareness of addiction, treatment pathways, and services
• Experience of assessing clients' needs in their homes and signposting to community supports.
• Experience of mobile working in a visiting support capacity.
