Connected Health in Ireland is seeking a Healthcare Supervisor to lead care teams in community homecare. You will mentor healthcare assistants, ensure high care standards, supervise spot checks, and support clients to live safely at home.

Formación

• Background in health or social care.
• Experience in homecare is preferred.
• QQI Level 5 Major Award in Healthcare Support or equivalent.
• Level 6 Supervisory Management qualification required.
• Full driving licence with access to a car.

Responsabilidades

• Undertake spot checks with healthcare assistants within the community.
• Carry out client reviews in relation to ongoing care.
• Carry out staff introductions in line with company policies.
• Responsible for staff shadowing – health & safety compliance.
• Rapid emergency response for new packages and discharges.
• Evaluate and monitor quality of care and report concerns to CCM.
• Support and develop care team.
• Update journal notes from clients’ homes.
• Deliver care plans, log sheets and gloves to homes.
• Deliver PPE as needed.
• Update online system documentation (reviews and spot checks).
• Plan weekly reviewed schedules with NOK and clients.
• Develop system for spot checks with healthcare staff.
• File paperwork and assist coordinators with gaps.
• Attend weekly check-ins with teams.

What we are looking for:

Experience & Qualifications

• Background in health or social care
• 1–2 years’ experience in homecare
• QQI Level 5 Major Award in Healthcare Support (or equivalent)
• Level 6 qualification in Supervisory Management
• Full driving licence with access to a car

Roles and Responsibilities:

Community:

• To undertake spot checks with healthcare assistants within the community
• To undertake client reviews in relations to the care they are continuing to receive
• To carry out the completion staff introductions in line with the company policies and procedures
• To be responsible for staff shadowing – ensuring Health & Safety policies and procedures
• To undertake emergency over - Rapid response (filling new packages and discharges)
• To evaluate and monitor the quality of the care delivered to clients and report to the CCM with concerns
• Support and assist with care team development
• Checking and collecting journal notes from clients houses
• Delivery of care plans, log sheets and gloves to clients homes when required
• Deliver of PPE

Office:

• To update online system documentation i.e., reviews and spot checks
• To develop a plan working week by booking in reviews (including confirming with NOK and clients)
• To develop a system of completing spot checks with Healthcare staff
• To be responsible for filing paperwork
• To provide support co-ordinators with uncovered/tasks in emergency if needed
• To attend weekly check ins with teams as required
