Job summary

Inspire Medical are partnering with an established primary care practice in Carlow to recruit an experienced General Practitioner to lead primary care and nursing home medical services. This role drives the Chronic Disease Management (CDM) Programme and supports expansion of HSE-funded initiatives.

A strong emphasis on clinical governance and team collaboration is required. You will provide comprehensive primary care to GMS and private patients, oversee CDM activities, and work closely with

Qualifications

• Specialist Registration with the IMC in General Practice.
• Medical degree (MB BCh BAO, MBBS or equivalent).
• Fully eligible to provide services under the General Medical Services (GMS) scheme.
• Minimum 5 years post-training experience in Irish General Practice, with proven expertise managing GMS patient populations.
• Demonstrated experience leading HSE Chronic Disease Management (CDM) programmes and chronic disease pathways.
• Full clean driving licence and access to personal transport for site visits.

Responsibilities

• Provide comprehensive primary care services to both GMS and private patients.
• Deliver ongoing clinical oversight and perform scheduled/ad-hoc medical visits to partner nursing homes.
• Lead and manage the HSE Chronic Disease Management (CDM) Programme, ensuring compliance and meeting targets.
• Implement HSE/GMS initiatives, preventative care, and integrated care pathways.
• Maintain thorough clinical documentation and participate in audits, quality improvement, and governance activities.
• Work alongside nursing, administrative, and allied health staff to ensure continuous, high-quality care.
