Job summary

Inspire Medical partners with an established primary care practice in Carlow to hire an experienced General Practitioner to lead the delivery of primary care and nursing home medical services.

This role drives the Chronic Disease Management (CDM) Programme and supports expansion of HSE-funded primary care initiatives, delivering high-quality patient care and clinical oversight across teams.

Qualifications

• Must hold Specialist Registration in General Practice with the Irish Medical Council (IMC).
• Medical degree in MB BCh BAO, MBBS or equivalent.
• Fully eligible to provide services under the GMS scheme.
• Minimum 5 years post-training experience in Irish General Practice with GMS experience.
• Evidence leading HSE Chronic Disease Management (CDM) programmes is required.

Responsibilities

• Provide comprehensive primary care services to GMS and private patients.
• Deliver clinical oversight and conduct visits to partner nursing homes.
• Lead the HSE CDM Programme and drive chronic disease pathways.
• Implement HSE/GMS initiatives, preventive care, and integrated care pathways.
• Maintain thorough clinical documentation and participate in audits and governance.

This key position involves driving the practice's Chronic Disease Management (CDM) Programme and contributing to the expansion of HSE-funded primary care initiatives.

Qualifications & Experience Required:

• Specialist Registration in General Practice with the Irish Medical Council (IMC) - this is non-negotiable
• Medical degree (MB BCh BAO, MBBS, or equivalent)
• Fully eligible to provide services under the General Medical Services (GMS) scheme
• Minimum 5 years post-training experience in Irish General Practice, with proven expertise managing GMS patient populations
• Demonstrated experience leading HSE Chronic Disease Management (CDM) programmes and chronic disease pathways
• Full clean driving licence and access to personal transport for site visits

Key Duties & Responsibilities:

• Provide comprehensive primary care services to both GMS and private patients
• Deliver ongoing clinical oversight and perform scheduled/ad-hoc medical visits to partner nursing homes
• Lead and manage the HSE Chronic Disease Management (CDM) Programme, ensuring compliance and meeting targets
• Implement HSE/GMS initiatives, preventative care, and integrated care pathways
• Maintain thorough clinical documentation and participate in audits, quality improvement, and governance activities
• Work alongside nursing, administrative, and allied health staff to ensure continuous, high-quality care
