Job summary

Curative Care in The Municipal District of Mullingar — Kinnegad is seeking a Family Service Coordinator to provide essential case management for children ages 0-3 with developmental delays. This role involves conducting screenings, organizing evaluations, and linking families with community resources.

The ideal candidate will have a Bachelor's Degree in a human service-related field and experience with children in the target age group. A commitment to bettering family lives and flexible working hours are part of what we offer at Curative Care.

Qualifications

  • Three years of experience as a Family Service Coordinator preferred.
  • One year of supervised experience working with families with special needs preferred.
  • Valid driver's license and reliable transportation required.

Family Service Coordinators provide case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Service Coordinators conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Service Coordinators link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge. Service Coordinators demonstrate excellent documentation, organization and customer service skills.

  • Knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed.
  • A Bachelor’s Degree in a human service related field (Human service related fields are those that include coursework in areas such as human development, psychology, social services or similar areas).
  • Three years or more of experience as a Family Service Coordinator (or equivalent case management position) preferred but not necessary. We will train the right person!
  • One (1) year of supervised experience working with families with special needs preferred.
  • A valid Wisconsin driver’s license, reliable transportation and car insurance are required.
  • Experience with children in the 0-3 age range and a love of working with them.
