Job summary

HSE Dublin North East Health Centre in Glasnevin is seeking a part-time Community Access Facilitator to support people with complex needs. The role is open to both HSE and non-HSE applicants and offers a flexi part-time contract of 19.5 hours per week minimum.

The successful candidate will work with a person-centred approach, develop and implement plans, communicate clearly in English, and may drive service vehicles if needed. A QQI Level 5 Health Care Qualification is essential.

• Fluent English and excellent communication skills.
