Job summary

Delta Centre CLG is seeking a Social Care Practitioner to provide day-to-day personal care and support for Residents. You will implement person-centred plans, review care protocols, and maintain strong communication to ensure residents’ rights and safety are upheld.

The role requires Level 7 qualifications in health-related disciplines, with experience in social care and a commitment to dignity, respect, and safeguarding vulnerable adults.

Qualifications

• Qualification at Level 7 or above in a health-related discipline.
• Experience with adults with intellectual disabilities preferred.
• Knowledge of National Standards for Adults with Disabilities.

Responsibilities

• Provide day-to-day personal care and support to Residents.
• Review and implement Person-Centred Plans for residents.
• Participate in reviews of care protocols and plans.
• Ensure residents’ rights, dignity, and safety are upheld.
• Communicate needs with appropriate systems and recordkeeping.
