Qualifications

• Requires a high school diploma or equivalent.
• Previous related experience in a healthcare setting preferred.

• Works well in a team and provides excellent customer service.
• Decision making and problem solving skills demonstrated.
• Cooperates professionally with staff and management.

Responsibilities

• Assist with planning and facilitating activities for individuals with disabilities.
• Provide personal cares and documentation of goal achievement.
• Accompany individuals on outings and provide excellent customer service.
• Maintain confidentiality and follow infection control procedures.
• Communicate clearly with individuals, families, and staff.
• Record changes in condition and report to supervisor.
• Assist with meal prep, transportation, and medication management as needed.
• Complete general upkeep of facility and用品 cleanliness.
