Job summary

The Rehab Group in Clonmel, Co Tipperary is seeking a Community Support Worker on a permanent contract for 35 hours per week to assist individuals with intellectual, physical and mental health challenges in the local community.

You will plan and deliver educational, recreational and occupational activities, monitor progress, and collaborate with families and the multidisciplinary team to promote inclusive community participation.

Qualifications

• QQI Level 5 in Social Care or equivalent.
• Experience supporting individuals with intellectual, physical and mental health challenges.
• Knowledge of local community resources is desirable.

Responsibilities

• Plan, develop, implement, monitor and review educational, recreational and occupational programs within the community.
• Ensure best practice aligned with current research to improve Quality of Life.
• Liaise with families, the multidisciplinary team and community personnel to enable accessible activities.

Community Support Worker Permanaent Contract, 35 hours The Mill Resource Centre Clonmel, Co Tipperary

Job Purpose

To assist in the delivery of individualized community support to a number of individuals with intellectual, physical and mental health challenges. Supporting individuals to be valued members of their local communities. Supporting individuals to optimize their emotional, social, occupational and learning experiences, through programmes offered within community. This role will also involve providing 1:1 support to an individual with social activities as well as working with groups in the community.

What we're looking for:

• Minimum of QQI Level 5 in Social Care, or equivalent
• Full Clean Drivers Licence
• Experience in the area of supporting individuals with intellectual, physical and mental health challenges.
• Knowledge and experience of local community resources is highly desirable

Your Responsibilities

• To plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community
• To ensure best practice in line with up to date research regarding Quality of Life improvements.
• To liaise with families, the multidisciplinary team and key community based personnel in the pursuit of accessible community activities for service users.

Salary

starting from point 1 € to point 11 €35,276.89 Pro Rata and is subject to negotiation depending on experience and qualification.
