Job summary

Depaul in Dublin is seeking a Community Integration Case Manager under Housing First principles. This permanent role offers a 35-hour work week, with a salary of €39,872.09 per year, reporting to the Deputy Manager.

The post involves coordinating complex support plans and promoting community integration for clients experiencing homelessness. Candidates should have at least 2 years in the homeless sector, with strong case management and inter-agency collaboration skills.

Qualifications

  • Deep understanding of Housing First principles.

  • Willingness for lone/off-site work when required.

Responsibilities

  • Assess needs with service users and develop workable support plans with regular reviews.
  • Promote social inclusion of Housing First clients in local communities.
  • Support clients to access local community opportunities and resources.
  • Utilise non-clinical community supports to improve wellbeing.
  • Develop individualised plans for development, training, work, and meaningful time use.
  • Monitor accommodation sustainment, including rent payments, using motivational approaches.
  • Establish positive relationships based on respect and independence.

Community Integration Case Manager - Housing First Dublin

Permanent Contract

35 Hours per week (Monday-Friday)

Salary - €39,872.09 per annum

Reporting to Deputy Manager

1. Key Areas of Responsibility
  • In partnership with service users, to assess their needs and develop workable support plans, with regular reviews on the progression of agreed goals.
  • Life Skills Assessment and Practical Support - In collaboration with the PSS, building on existing life skills interventions to empower individuals to live independently.
  • Promoting the social inclusion of Housing First clients into local communities through effective use of community development principles and practices.
  • To support and facilitate clients to access a range of opportunities in their local community
  • Connecting people to non-medical sources of support or resources in their community.
  • Utilise the HSE social prescribing program, which addresses health influenced by social factors like poverty and isolation, by connecting individuals to non-clinical community supports that enhance overall well-being.
  • To develop individualised plans around personal development, accessing training, employment and enjoyable/productive meaningful use of time based on the on unique attributes, preferences and aspirations of the individual.
  • In partnership with service-users assess their post-settlement, tenancy sustainment and social integration needs and develop & implement post-Settlement Support Plans.
  • Monitoring of support requirements of participants to sustain their accommodation i.e. rent payments etc. and work with the participant utilising motivational and recovery orientated techniques to ensure sustainment of accommodation.
  • Ensure support plan outcomes are focused on the development of a socially valued role.
  • Support with accessing local groups and activities for example, sporting, music, arts and cultural activities, green space or gardening, debt advice or budgeting, physical activity and leisure, library membership, volunteering, housing maintenance and more
  • To establish and maintain positive, trusting relationship with the person being supported, that is based on respect, equality and that promotes independence.
  • To ensure all person specific entitlements and opportunities are utilised to their maximum benefit.
  • To develop strategic links, collaborations and robust pathways with relevant external stakeholders and agencies focusing on various levels of participation & sustainable social role development.
  • Develop and maintain positive working relationships with all stakeholders and develop productive positive working networks within the wider community
  • Lead on developing local forums and elevate issues as appropriate.
  • Ensure Depaul’s Good Neighbourhood Policy is being implemented within the service and support service users in adhering to policy expectancies
  • To ensure a health, safety and welfare of self, colleagues, and clients is a priority at all times.
  • To understand the barriers that exist to integration and explore appropriate mechanisms to overcome these barriers.
  • Assisting in addressing any issues / concerns / grievances / complaints that arise in interactions with public services;
  • Delivering on DRHE Case and Case Management Protocols.
  • Building relationships with partners and identified support agencies
  • Commitment to the principles of Housing First.
  • Adherence to Organisational Health & Safety Policies
  • Strive to work creatively and take initiative to assist the service in achieving its core objectives and ensuring that the needs of service users are met.
  • Participation in Out of Hours On Call Support Rota
2. Person Requirements
  • Must possess a minimum of 2 years of experience in the homeless sector or a related field, with at least one year dedicated to coordinating support plans for service users with complex needs
  • Have a deep understanding of the principles of Housing First
  • Have a deep understanding of the root causes of homelessness and the challenges faced by individuals at risk.
  • To research and educate the staff team on Community Services available to participants.
  • To Promote Depaul Housing First externally; providing talks to external services as required.
  • Candidates should have experience collaborating with various agencies in both voluntary and statutory sectors
  • Experience in lone or off-site working is desirable
  • Clear comprehension of the case management approach is crucial for success in this role.
  • To attend and participate in team meetings and staff team days.
  • To attend all mandatory training and any training deemed relevant to service delivery.
  • To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
  • To respect service users’ rights to confidentiality in our relationship with them as per the Depaul Confidentiality policy and code of conduct.
