The Health Service Executive is seeking a Community Connector to support WALK's day services in North and South Dublin. The role focuses on person-centred practice and helping people live independently within their chosen community.

You will work as part of a team to deliver outcomes in employment, education/training and personal development, with a strong emphasis on flexibility and high-quality support.

Qualifications

• Demonstrated commitment to person-centred practice.
• Experience in disability/day services is desirable.
• Ability to work flexibly as part of a team.

Responsibilities

• Provide support to people taking part in WALK’s Day support services to enable them to build a lifestyle and live their life as part of their chosen community.
• Support individuals to make and exercise choice and participate in the community.
• Be an essential part of a team focused on outcomes in employment, education/training and personal development.
