Job summary

St John of God Dublin South East Services provides a network of supports for children and adults with intellectual disabilities in South Dublin. The successful candidate will work as a Community Access Facilitator to help service users achieve personal goals and participate in meaningful activities within the community.

The role requires a Fetac/QQI Level 6 qualification in Social Care/Training/Education/Teaching, a commitment to person-centred planning, and the ability to support service users

Qualifications

• Ability to support service users to achieve personal goals and independence through person-centred planning.
• Excellent communication, organisational, interpersonal and teamwork skills.
• Commitment to a Person Directed Planning approach.
• Must hold a full valid manual driving licence.

Responsibilities

• Support service users to achieve personal goals and independence through person-centred planning.
• Facilitate meaningful, goal-driven activities that promote learning and community participation.
• Provide respectful support with personal care, communication, and daily living needs.
• Ensure compliance with health, safety, and safeguarding procedures.
• Maintain accurate records and documentation in line with policy.
• Collaborate effectively with colleagues and external professionals.
• Uphold confidentiality, equality, and professional standards at all times.
• Build links with community resources to enhance opportunities for service users.
• Participate in training and continuous professional development.
• Contribute to service quality and improvement through teamwork and initiative.
