Job summary

Sunbeam House Services in Ireland is seeking a Community Access Facilitator to empower adults with disabilities to live meaningful, self-directed lives within their communities. You will work as part of our Adult Disability Day Services team, supporting personal goals, inclusion and participation.

The role emphasizes person-centred planning, human rights, and collaboration with families, circles of support and community organisations to promote education, work, and social connections.

Qualifications

• A relevant QQI Level 5 qualification (or equivalent) in Health & Social Care, Community Care, Community Development, Youth Work, Healthcare Support or another relevant healthcare discipline.
• Relevant experience working in community, social care, disability services, employment support or similar settings is desirable.

Responsibilities

• Support individuals to identify and achieve personal goals through person-centred planning.
• Promote community inclusion, active citizenship and independence.
• Assist people to access education, training, volunteering and employment opportunities.
• Help individuals develop social networks, friendships and meaningful community connections.
• Support self-advocacy, informed decision-making and positive risk-taking.
• Work collaboratively with families, circles of support, multidisciplinary teams and community organisations.
• Provide practical and personal support when required, always maintaining dignity, respect and choice.
• Act as a key worker and help ensure high-quality outcomes for the people you support.
