RecruitNet International Ltd in Dublin is hiring a Clinical Nurse Manager (CNM) - Patient Flow for a Nursing Home in County Dublin. You will manage referrals, conduct pre-admissions, coordinate discharges, and lead a multidisciplinary team to ensure high-quality care.

The role requires strong leadership, HIQA knowledge, and active collaboration with therapy and medical teams. Applicants must have at least 3 years post-registration experience, Level 6 management qualification, NMBI registration,

Qualifications

• Minimum 3 years' post-registration clinical experience.
• Management qualification Level 6 or higher.
• Current NMBI registration and live on the register.
• Experience in older people care or nursing home settings.
• Knowledge of HIQA Standards and Health Act Regulations 2013.
• Excellent English, with eligibility to work in Ireland.
• Strong communication and teamwork skills.

Responsibilities

• Manage patient referrals from hospitals and liaise with referral teams.
• Conduct pre-admission assessments for suitability.
• Work with ADON on discharge plans and dates for admissions.
• Coordinate admission of accepted referrals to the centre.
• Act as case manager with the Patient Flow team as needed.
• Ensure compliance with healthcare policies and standards.
• Maintain accurate assessment records and referral documents.
• Participate in daily handovers and MDT meetings.
• Provide nursing unit cover to support operations.
• Liaise with doctors on admissions and prescriptions.
• Monitor outpatient appointments and follow ups.
• Coordinate admissions and discharges efficiently.
• Refer patients to Medical Social Worker when appropriate.
• Maintain communication with referral sources and community services.
