Job summary

Mowlam Healthcare is seeking a Clinical Nurse Manager, Patient Flow for Leopardstown Care Centre in Murphystown, Dublin. You will support and deputize the ADON, Patient Flow to ensure safe management of referrals and discharge planning.

The role requires 3+ years post-registration experience, NMBI registration, and a managerial qualification; HIQA standards knowledge and strong communication are essential. You will work as part of a team with a patient-centered approach.

Qualifications

• 3 years post-registration experience in a relevant field and experience in managing patient caseload.
• Hold a recognised Managerial Qualification (minimum level 6 on the National Framework of Qualifications).
• Registered on the NMBI live register.
• Clinical experience working in older persons care or nursing home care.
• Working experience of HIQA Standards and Health Act Regulations 2013.
• Fluent in English language (verbal and written) and be eligible to work in Ireland.
• Strong communication and listening skills.
• Ability to work well as part of a care team.
• Caring and compassionate and be passionate about providing high quality person-centred care.
• Have a basic knowledge of IT systems such as excel spreadsheets.

Responsibilities

• Receive patient referrals from key sources and liaise with referral teams.
• Complete pre-admission assessments to ensure suitable placement.
• Coordinate discharge planning and set anticipated discharge dates.
• Arrange admissions for accepted referrals to Leopardstown Care Centre.
• Act as case manager for patients as required.
• Adhere to Mowlam Healthcare policies and procedures.
• Maintain clear assessments and records for referrals.
• Attend daily handovers and weekly MDT meetings.
• Provide cover for units as necessary.
• Liaise with doctors about admissions and ensure paperwork is prepared.
• Follow-up on outpatient appointments as necessary.
• Facilitate patient admissions and discharges.
• Refer patients to Medical Social Worker as required.
