Dovida Ireland is seeking a Care Manager to lead client care plans, conduct risk assessments, and supervise a team of caregivers across Ireland. You will ensure care plans meet HSE standards and QA requirements, and build strong relationships with clients, families and healthcare partners.

Applicants should hold QQI Level 5 Social Healthcare, have 2–3 years in home care, be computer literate, and possess a full clean driving licence with access to a car.

Competenze

• QQI Level 5 Social Healthcare qualification is required.
• 2–3 years' experience in home care or health care is necessary.
• Must have computer literacy and a valid driver's licence with access to own car.

Mansioni

• Conduct consultations and prepare care plans.
• Conduct risk assessments in the home and mitigate risks.
• Monitor, mediate and manage client and caregiver concerns.
• Carry out QA visits, review care plans, and conduct service enquiries as needed.
• Ensure care aligns with HSE ISO and franchise standards and local policies.
• Mentor and supervise a large team of caregivers.
• Develop and maintain client relationships and liaise with health professionals.
