Job summary

OC Recruitment is seeking an Aftercare Service Coordinator for a well-established health and social care provider in Cork. You will lead the Aftercare team, supervise staff, and support individual care plans for young adults aged 18-23 to promote independence and successful adulthood.

The role requires a Level 7 degree in Social Care or related discipline, CORU registration, and experience in social care settings.

Qualifications

• A Level7degree or higher in Social Care or a relevant discipline.
• CORU registration as a Social Care Worker.
• Experience in social care, aftercare, disability, health or related service.

Responsibilities

• Leading, motivating and supporting the Aftercare team.
• Providing staff supervision, coaching and development.
• Supporting the implementation and review of individual Aftercare Plans.
• Liaising with families, referral agencies and multidisciplinary professionals.
• Promoting independence, participation and rights of young adults.
• Supporting safeguarding, risk management and quality standards.
• Monitoring records, documentation and service compliance.
• Leading team meetings and supporting service improvement.
• Deputising for the Aftercare Manager when required.

OC Recruitment is delighted to be recruiting an Aftercare Service Coordinator on behalf of our client, a well-established health and social care provider in Cork. This is an excellent opportunity for an experienced Social Care professional who enjoys leading and supporting teams while making a real difference in the lives of care-experienced young adults aged 18-23. You will play a key role in the day-to-day delivery of the service, supporting young adults in building independence, achieving their goals, and successfully transitioning into adulthood.

Key responsibilities will include:

• Leading, motivating and supporting the Aftercare team

What Were Looking For

• A Level7degree or higher in Social Care or a relevant related discipline
• CORU registration as a Social Care Worker
• Experience working within social care, aftercare, disability, health or a related service
• Previous leadership, coordinator or management experience is highly desirable
• Experience supervising or supporting staff would be an advantage
• Strong leadership, communication and organisational skills
• Good knowledge of safeguarding, relevant legislation and person-centred practice
• A full driving licence
• Eligibility to work in Ireland
