Job summary

The Rehab Group is seeking a Community Support Worker for a 6 month fixed term contract, 19.5 hours per week, based in Eyre Square, Galway. You will assist in delivering individualized community support to people with intellectual, physical and mental health challenges, helping them participate in local community activities and develop essential life skills.

Qualifications

• Minimum QQI Level 5 in Health and Social Care or equivalent is desirable.
• Willingness to participate in Rehab Care training programme and external training opportunities.
• Experience in supporting individuals with intellectual, physical and mental health challenges.
• Track record of supporting individuals to identify and experience meaningful experiences in their local community.
• Experience of promoting self-advocacy.
• Experience in the provision of personal care support.
• Experience in supporting community independence.

Responsibilities

• Plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community.
• Document information, update records, and complete necessary paperwork for communication and accountability purposes.
• Ensure all interactions with service users are dignified, respectful and inclusive of choice.
• Deliver a quality service by implementing and reviewing quality monitoring systems and assisting with daily living tasks as needed.
• Support service users to integrate into their local community and access local resources.
• Ensure privacy and confidentiality of service users at all times.
• Comply with health and safety legislation and follow risk assessments.
