Qualifications

• Valid Washington State RN license.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum three years of clinic experience as an RN.
• Experience with electronic health records (EHR).
• Experience with process improvement is a plus.

Responsabilités

• Performs independent visits under standing order procedures, including health assessments and patient education.
• Collaborates with mobile medicine, outreach, and primary care teams on population health initiatives.
• Supports health equity initiatives to reduce disparities for marginalized populations.
• Coordinates care and creates care plans for unengaged patients to improve health outcomes.
• Provides patient, family and caregiver education, including motivational interviewing.
• Addresses social determinants of health with community resources.
• Performs other duties supporting CHAS Health Mission and Core Values.
